This plan is designed to be a low-cost benefit that helps pay for out-of-pocket expenses while employees are confined in the hospital or are outpatients. The coverage, which "fills the gaps" left by many medical insurance policies, provides: Outpatient benefits (up to a maximum) for emergency room treatment, diagnostic testing, and outpatient surgery for conditions separated by 90 consecutive days. Physician benefits for outpatient treatment at an emergency room, physician's office, or clinic. In-hospital benefit for the difference between actual inpatient expenses and the amount paid by other medical plans for the out-of-pocket covered charges, up to the maximum benefit selected per confinement.
