Critical Conversations on the Changing Health Environment: Population Health
A focused dialogue on the need for major shifts in the way post-acute care is delivered.
As health care organizations take on more responsibility for the health of the populations they serve, providers are grappling with a host of questions. How do you best assemble teams for integrated care? How do you maintain contact with patients after they are discharged? How do you establish collaborative relationships within the community to provide the support patients need? Most of all, how do you prevent unnecessary readmissions?
Questions such as these point to the need for major shifts in the way post-acute care is delivered. Implications abound — related to patient relationships, workforce, culture, data integration, cost control, physician education and many other issues.
In 2015, the American Hospital Association (AHA) brought together health providers, health care field partners and AHA leaders for collaborative discussion to help all anticipate and support the next iteration of care. AHA Solutions, Inc., a subsidiary of the AHA, sponsored the two “Critical Conversations” events.
The discussions examined the challenges of population health management from a variety of perspectives, including clinical integration and IT, communication, staffing, culture, patient flow, care transition and others.
