With the help of proprietary automation, the solution enables clinicians to manage the care of patients with serious medical conditions by prescribing personalized Information and remote patient contact pathways apps. This prescribed content educates patients on their care and enables them to feed information (such as treatment adherence for congestive heart failure, or post-surgery progress and recovery) back to physicians in real time and between clinic visits.

Patients use their own PCs, tablets or mobile phones to securely access the platform’s content. They can also use the platform to communicate with their clinicians by reporting on their progress, asking questions about their care plan and providing feedback regarding their satisfaction with their care. As patients progress, they complete health literacy quizzes and questionnaires related to their progress and care experience to earn Personal Learning and Engagement credits redeemable for discounts, co-pay coupons or other health care benefits. As these incentive-based credits motivate patients to learn and improve health behavior and outcomes, they allow physicians to measure patient activation and follow up on patients’ progress.

Hospital training managers can also use the platform to train and engage clinicians and workforce by creating effective personalized training programs, tracking completion and measuring the impact of every training module on organizational performance objectives.
