The Centers for Medicare & Medicaid Services (CMS) reports that nearly 18% of Medicare patients are readmitted within 30 days of discharge, resulting in approximately 2 million readmissions each year, which costs the Medicare program roughly $15 billion annually. In addition to some already-in-progress pilot programs and other initiatives focused on reducing readmission rates, the Administration on Aging recently awarded 16 grants to states to continue developing evidence-based care transitions models.

While focusing on ways to reduce readmissions, hospitals must also follow a consistent admission review process to ensure the appropriateness of all readmissions.
