Helen Portalatin is a Board Certified Nurse Practitioner with a Masters of Science in Nursing from Columbia University. She has over 20 years of experience in community nursing and public health. She is currently the Director of the CARETEAM Program (Community and Residential Extenders for Transitions, Evaluation, and Management) at Crystal Run Healthcare in Middletown, NY.

Careteam provides transitional care for Crystal Run patients discharged from the hospital to home, which includes Nurse Practitioner home visits, telemonitoring, and care management. Careteam also provides Primary Care services in the home. She participates in a number of community initiatives with local skilled nursing facilities, home care agencies, and other community stakeholders that aim to reduce avoidable hospital admissions and lower total cost of care.
